The FPS has removed all document models that an external prevention service must use from the appendix of the respective legal texts. The current way of working consists of simply publishing the documents on the FPS website.
One can thus find a first series of documents on health surveillance: https://werk.belgie.be/nl/themas/welzijn-op-het-werk/het-gezondheidstoezicht-op-de-werknemers and then click “documentation” and “procedures and forms”. There, since 4/12/2024, one can find:
- Form for the health assessment (DOCX, 79 KB)
- Explanatory note for filling in a health assessment form (PDF, 227.43 KB)
- Form for the reintegration assessment (DOCX, 66.83 KB)
- Form for the determination of definitive incapacity in the context of the special procedure for medical force majeure (DOCX, 69.98 KB)
- Form of appeal against the decision of the prevention adviser-occupational physician (DOCX, 68.55 KB)
Premed will therefore put these new documents into use as soon as possible. To do so, Premed must implement a number of internal IT changes, which go beyond merely deploying a new template. The FPS has therefore informed us that we can count on a transition period.
The novelties speak for themselves and should not cause any problems once the new documents have been put into circulation. It would lead us too far to explain them in advance.
