Every employer who employs staff in Belgium is obliged to join an External Service for Prevention and Protection at Work (ESPPW), unless they have an internal service that fully meets the legal requirements.

Who can join Premed?

Employers can join if they:

  • Have at least one employee
  • Are not yet affiliated with another external service
  • Want to switch from another external service (taking into account the legal notice period of the current contract)

What do you pay?

The contribution an enterprise pays is determined based on three criteria:

  1. The main activity of the organisation
    This concerns the activity performed by the majority of employees within the enterprise. Annex II.3-1 of the Codex on Well-being at Work provides an overview of activities per tariff group. As additional information, the corresponding NACE codes are also listed.
  2. The size of the organisation
    Enterprises are divided into two categories:
    • Enterprises with a maximum of five employees
    • Enterprises with more than five employees
  3. Number of employees

Flat-rate company

Who?

According to legislation, it is mandatory to engage a prevention adviser of Level I or II, depending on factors such as the number of employees, the nature of the activities, and the associated risks. You are a flat-rate company if your company does not employ a prevention adviser of Level I or II.

  • A flat-rate company is typically a company with fewer than 200 employees and no trained prevention adviser of level 1 or 2.

Contribution?

You pay Premed an annual flat-rate amount per employee. The size of this amount can be found in the table below.

What can you expect from Premed for this contribution?:

  • Performing a company visit
  • Delivering motivated policy advice
  • Performing certain medical examinations
  • Participating in risk analyses (such as VDU work)
  • Certain assignments regarding psychosocial support
  • Providing assistance in case of serious occupational accidents
  • Attending a CPBW
  • Information: many tips & tricks & documentation about the welfare domains

For projects outside this list, a quote will be drawn up. This quote is made based on the number of hours we will work on a project. You then pay € 154.78 per hour.

Prevention Units (PU) company

Who?

A PU company has:

  • An internal prevention adviser with Level I or II training
  • More than 20 employees
  • Or the company is affiliated with an intermunicipal body (and therefore does not employ a prevention adviser level 1 or 2 itself)

Contribution?

You pay based on prevention units, which are used to deploy services. The total amount (= number of employees x contribution per employee) is converted into these prevention units. 

For 2026, 1 PU = € 201.89

In the spending of these PUs, there is a legally determined order that every company must respect:

  1. Mandatory medical examinations
  2. Certain interventions psychosocial aspects

Remaining units can be spent according to your own needs such as training, measurements, ...

Premed keeps you informed of the consumption of your prevention units via the Extranet. On the client portal, you have transparent insight into the consumption of your units.

If the prevention units are not used up at the end of the year, you can transfer them indefinitely to the following year.
